I agree that my child will abide by normal school rules and refrain from smoking (vaping incl), use of alcohol or any other substances. Any child found in a situation where these substances are present, can be sent home at the parents expense

Indemnity clause

BY SUBMITTING THIS FORM : I indemnify and hold the University of Pretoria, Die HS Menlopark, World of Squash as well as its employees, representatives and consultants harmless against all loss, costs, damage and disbursements of whatever nature, including legal costs and disbursements relating to any damage that may be suffered, however caused, directly or indirectly, arising from my participation in or involvement with any of the above sports activities.

In loco Parentis clause

BY SUBMITTING THIS FORM : I hereby authorize the Head coach, Elizabeth S Mackenzie ID : 6105280150088 and participating qualified coaches, to act in loco parentis in respect of my child, for the duration of the Nat u13 High Performance camp . Furthermore, I grant them the authority to take any reasonable action necessary to ensure the safety, health and well-being of my child, including authorising urgent medical attention or hospital treatment if I cannot be reached in an emergency.
